Real Time Updates on Patient Activity

SHIIP enables you to receive real-time notifications during specified patient activity. Users can choose to be notified when a patient is visiting/leaving a hospital, or when a certain page on a patient’s Coordinated Care Plan is updated.

Users may apply notification settings to any of their patient lists (lists that are manually maintained by users) as well as their organization’s practice list. A full list of when you can be notified is below:

  • Hospital inpatient admission
  • Hospital inpatient discharge
  • Hospital inpatient discharge with LACE score greater than a specified value
  • Emergency department visit
  • Discharged from emergency department visit
  • Discharged from emergency department visit with HARP score greater than a specified value
  • CCP Information page was updated
  • CCP Identifiers page was updated
  • CCP Care Team page was updated
  • CCP Care Consent & ACP page was updated
  • CCP My Health page was updated
  • CCP More About Me page was updated
  • CCP Goals and Action Plan page was updated
  • CCP Medication Coordination page was updated
  • CCP Allergies page was updated
  • CCP Medications page was updated
  • CCP Health Assessments page was updated
  • CCP Last Hospital Visit page was updated
  • CCP Palliative Approach to Care was updated

In addition to the above notification settings, users may also choose if they would also like to be notified via email.
