Real Time Updates on Patient Activity
SHIIP enables you to receive real-time notifications during specified patient activity. Users can choose to be notified when a patient is visiting/leaving a hospital, or when a certain page on a patient’s Coordinated Care Plan is updated.
Users may apply notification settings to any of their patient lists (lists that are manually maintained by users) as well as their organization’s practice list. A full list of when you can be notified is below:
- Hospital inpatient admission
- Hospital inpatient discharge
- Hospital inpatient discharge with LACE score greater than a specified value
- Emergency department visit
- Discharged from emergency department visit
- Discharged from emergency department visit with HARP score greater than a specified value
- CCP Information page was updated
- CCP Identifiers page was updated
- CCP Care Team page was updated
- CCP Care Consent & ACP page was updated
- CCP My Health page was updated
- CCP More About Me page was updated
- CCP Goals and Action Plan page was updated
- CCP Medication Coordination page was updated
- CCP Allergies page was updated
- CCP Medications page was updated
- CCP Health Assessments page was updated
- CCP Last Hospital Visit page was updated
- CCP Palliative Approach to Care was updated
In addition to the above notification settings, users may also choose if they would also like to be notified via email.
