SHIIP receives data from several sources. All SHIIP data is contained and transported within the eHealth ONE network.
Primary Care Providers
With secure access, primary care providers have the ability to contribute data via Coordinated Care Plans.
Hospitals
Hospitals contribute SHIIP’s most prominent data – ADT, DAD, and NACRS. Here is a description of each:
- Discharge Abstract Database (DAD): Captures administrative, clinical, and demographic information on discharge. This includes deaths, sign-outs, and transfers.
- National Ambulatory Care Reporting System (NACRS): Supplies SHIIP with valuable hospital-based and community-based abulatory care data. This includes day surgery, outpatient / community-based clinic data, and emergency department data. This data is collected at the time of service, but methods of this collection may vary between facilities.
- Admission, Discharge, and Transfer (ADT): Captures patient demographic and administrative information when a patient is admitted, discharged, or transferred.
Combined, the following data is supplied to SHIIP:
- A patient’s identifying information (names, health card number, date of birth, etc)
- A patient’s attending / referring / consulting physician
- The patient’s location (floor, room, bed) if available
- Admission date/time, discharge date/time
- Patient’s chief complaint and other issues
- Any diagnoses codes assigned to the patient

Community Support Services
These services within the South East LHIN primarily utilize one client management system, NesdaTrak. SHIIP integrates with NesdaTrak to populate patient records with community support data. The following information is captured by NesdaTrak:
- Patient details (name, address, phone number, etc)
- Service name
- Service notes (if available)
- Service start / end dates
- Assessment scores (AUA, CSI, MAPLe)
The following community support agencies send information to NesdaTrak (and to SHIIP):
- Community Care for South Hastings – Provides services and programs for seniors and adults with physical disabilities. Areas serviced include Belleville, Thurlow, Tyendinaga, and Deseronto.
- Community Care for Central Hastings – Provides services and programs for seniors and adults with physical disabilities. Areas serviced include Madoc, Marmora, Stirling, Tweed and surrounding area.
- Prince Edward County Community Care for Seniors – Provides services and programs for seniors living in Prince Edward County.
- Alzheimer Society Lanark Leeds Grenville – Provides resources, education, support, and counselling to individuals with Alzheimer’s Disease. Areas serviced include the Lanark, Leeds, and Grenville counties.
- Northern Frontenac Community Services – Provides a variety of services to both adults and children in the Northern Frontenac community.
- Community Home Support – Lanark County (Perth) – Provides services and programs for seniors, adults with physical disabilities, and individuals that have been diagnoses with a life threatening / terminal illness.
- Community Home Support – Lanark County (Smiths Falls) – Provides services and programs for seniors, adults with physical disabilities, and individuals that have been diagnoses with a life threatening / terminal illness.
- Community & Primary Health Care – Provides services and programs to seniors and their families. Areas serviced include Kingston, Lanark, and Leeds and Grenville.
- Land O’Lakes Community Services – Provides services and programs to seniors, adults with disabilities, and women in abusive situations. Areas serviced include the Ontario highlands.
- Frontenac Transportation Service – Provides affordable transportation services to individuals requiring a ride to appointments or other programs and services.
- Tyendinaga Home Support Program – Provides services and programs for seniors and adults with physical disabilities. Areas serviced include the Tyendinaga Mohawk Territory.
- Community & Primary Health Care (LL) – Provides services and programs to seniors and their families. Areas serviced include Kingston, Lanark, and Leeds and Grenville.
- Alzheimer Society of Hastings-Prince Edward County – Provides resources, education, support, and counselling to individuals with Alzheimer’s Disease. Areas serviced include the Lanark, Leeds, and Grenville counties.
- Hospice Quinte – Provides palliative care to terminally ill individuals as well as support to their caregivers. Areas serviced include Belleville, Quinte West, and South Hastings.
- Southern Frontenac Community Services Corporation – Provides services and programs that aim to improve and maintain the quality of life for residents in the Southern Frontenac area.
- Hospice Kingston – Provides palliative care to terminally ill individuals as well as support to their caregivers. Areas serviced include the Kingston area.
- Alzheimers Society of Kingston – Provides resources, education, support, and counselling to individuals with Alzheimer’s Disease. Areas serviced include the Kingston, Frontenac, Lennox and Addington areas.
- Community Care North Hastings – Provides services and programs for seniors and adults with physical disabilities. Areas serviced include the North Hastings area.

Addictions and Mental Health
The Addictions and Mental Health (AMH) agencies within the South East LHIN currently utilize EMHWare for all clinical and administrative information. SHIIP integrates with EMHWare to populate the patient record with services provided by AMH agencies. The following AMH agencies contribute to SHIIP:
