Primary Care Providers

With secure access, primary care providers have the ability to contribute data via Coordinated Care Plans.

Hospitals

Hospitals contribute SHIIP’s most prominent data – ADT, DAD, and NACRS. Here is a description of each:

  • Discharge Abstract Database (DAD): Captures administrative, clinical, and demographic information on discharge. This includes deaths, sign-outs, and transfers.
  • National Ambulatory Care Reporting System (NACRS): Supplies SHIIP with valuable hospital-based and community-based abulatory care data. This data is collected at the time of service, but methods of this collection may vary between facilities.
  • Admission, Discharge, and Transfer (ADT): Captures patient demographic and administrative information when a patient is admitted, discharged, or transferred.

Combined, the following data is supplied to SHIIP:

  • A patient’s attending / referring / consulting physician
  • The patient’s location (floor, room, bed) if available
  • Admission date/time, discharge date/time
